- a completed copy of the Affidavit form with notarized signature
- proof of a valid non-commercial Texas driver's license
- the court administrative fee
- a copy of your insurance policy
- a copy of your Driving Record, if required
- Submit your certificate of completion to the course within 90 days once permission to take the course is granted
