- You must request permission to take the course by the appearance date shown on the ticket, in person or by mail, and submit:
- A completed copy of the Affidavit form with notarized signature
- Proof of a valid non-commercial Texas driver's license
- The court administrative fee
- A copy of your insurance policy
- A copy of your Driver Record, if required
- You must admit guilt or sign a statement that they do not contest their citation
- You may NOT have had a ticket dismissed within the previous twelve months
- You may NOT have been cited for driving 25 mph or more over the posted speed limit
